Become A Patient!BHA offers a variety of programs and services and always welcomes new patients. Whether you live in one of our neighborhoods, or simply would like to get established with a physician, BHA can provide you with a medical home. You may register at any clinic location.
Patient Registration Schedule
Registration process is required before medical appointment is scheduled- Monday through Friday 8:00 AM – 11:30 AM & 1:00 PM – 4:30 PM
If applying for the Sliding-Fee Scale, please bring the following:
- Picture ID - or other proof to confirm Brevard County, Florida residency
- Social Security Card - if possible
- Proof of monthly income - for the last months for all related household members, such as
- Paycheck Stubs
- Social Security Income
- Bank Statements
- W2 Statements
- Federal Income Tax Return - the patient's most recently filed Return is required to apply for assistance with medications.
